Every command defaults to --mode sim; real motion refuses with CONFIRM_REQUIRED without
--confirm. Output is the same envelope the agent tools return: {"ok": true, "result": ...} or
{"ok": false, "refusal": {"code", "value", "cause", "remedy"}}.
